01Agreement to these terms
These terms apply to transportation services provided by ONTIME MEDICAL MOBILITY LLC (“we,” “us,” or “our”) and to our text message program. By using our services or receiving our messages, you agree to these terms.
02Our service
We provide non-emergency medical transportation — door-to-door rides to and from medical appointments in Milwaukee and southeastern Wisconsin. Our rides are arranged and authorized through health plans, Wisconsin Medicaid, and transportation brokers.
We do not provide medical care. Our drivers are not medical professionals. We do not provide ambulance service, and our vehicles are not equipped or staffed for medical emergencies.
03Not for emergencies
If this is a medical emergency, call 911
Do not call us for an emergency and do not wait for one of our vehicles. Call 911 immediately.
04Who can ride
Our rides must be authorized in advance by a health plan, Medicaid, or a transportation broker. We cannot provide an unauthorized ride. If you are not sure whether your ride is covered, contact your health plan or the transportation phone number on your plan card before your appointment.
Minors may ride when the trip is arranged by a parent, guardian, or health plan. An escort may ride with you when it is authorized and we have been told in advance, since seating is limited.
05Booking and confirmation
Your health plan or broker schedules the trip and sends us the details. We then contact you to confirm your pickup time and address before the ride.
Please confirm your ride when we contact you. If we cannot reach you, we will try again by phone. If we still cannot reach you, we may have to release the trip, because we cannot send a vehicle to an unconfirmed pickup.
If the details we have are wrong — the time, the address, the vehicle type — tell us when we confirm, or call us at (414) 555-0100. We will help sort it out with your plan or broker.
06Text message program
Our text messaging program is called ONTIME MEDICAL MOBILITY LLC Ride Reminders. Full details are in our Text Message Terms. In summary:
- How you join. You agree during a phone call with our team. We do not enroll you automatically.
- What you receive. Reminders confirming the date and pickup time of an upcoming ride.
- Frequency. Up to 4 messages per trip. Message frequency varies.
- Cost. Message and data rates may apply.
- To stop. Reply STOP to any message.
- For help. Reply HELP, or call (414) 555-0100.
- Not required. You do not have to agree to text messages to receive transportation. If you decline, we will confirm your rides by phone.
Carriers are not liable for delayed or undelivered messages. Message delivery is not guaranteed.
07Cancellations and changes
If your appointment changes or you no longer need your ride, tell us as soon as you can so the vehicle can be used by someone else. Call (414) 555-0100.
If you ask for a different pickup time or address, we will not change the trip on our own. A member of our staff will review it and contact you, because the change may need to be authorized by your plan or broker first.
If a vehicle arrives and you are not available, the trip may be recorded as a no-show. Repeated no-shows can affect whether your plan or broker authorizes future trips, so please let us know if your plans change.
08During your ride
- Please be ready at your pickup location at the scheduled time. We can only wait a limited time before it affects the next passenger.
- Seat belts must be worn, and wheelchairs must be secured by the driver.
- Service animals are welcome at no extra charge. Please tell us in advance so we can send an appropriate vehicle.
- Smoking, alcohol, and illegal substances are not permitted in our vehicles.
- Our drivers may end a trip if they are threatened or if a passenger’s behavior makes the ride unsafe.
09Cost and billing
When your ride is authorized by a health plan, Medicaid, or a broker, there is no cost to you. We bill the authorizing organization, not the passenger. If you are ever asked for payment, please call us at (414) 555-0100 before paying anything.
Message and data rates from your mobile carrier, if you receive our text messages, are your responsibility and are set by your carrier.
10Limitation of liability
We work to arrive on time, but traffic, weather, and vehicle problems can cause delays. We are not responsible for missed appointments or other losses caused by delays outside our control.
To the extent permitted by law, our responsibility for any claim relating to a trip is limited to the amount we were paid for that trip.
Nothing in these terms limits any rights you have under Wisconsin law or under your health plan or Medicaid coverage.
11Accessibility
We provide wheelchair-accessible vehicles and drivers trained to assist passengers with mobility needs. If you need assistance, or if you use a mobility device, tell us when we confirm your ride so we send the right vehicle and the right driver.
We want this website to be usable by everyone. If any part of it is difficult for you to read or use, call (414) 555-0100 and we will help you directly.
12Privacy
Our Privacy Policy explains what information we collect and how we handle it, including our text message privacy practices. It is part of these terms.
13Governing law
These terms are governed by the laws of the State of Wisconsin. Any dispute will be handled in the state or federal courts located in Wisconsin.
14Changes to these terms
We may update these terms. When we do, we will change the date at the top of this page. Continuing to use our services after a change means you accept the updated terms.
